What Is the Degree of Burn Model Used For?

The familiar depth scale runs from first-degree (superficial) burns, which affect only the epidermis and leave the skin red and painful without blisters, through second-degree (partial-thickness) burns, which reach into the dermis and raise blisters, to third-degree (full-thickness) burns, which destroy the whole skin and may look white, leathery or charred — often with little pain, because the nerve endings are destroyed. Deeper burns into fat, muscle or bone are sometimes called fourth-degree. Forensic medicine texts also teach older schemes such as Dupuytren’s and Wilson’s classifications, and the area burned is estimated separately, for example with the rule of nines.

A model makes the link between surface appearance and depth easy to see. Nursing and first-aid courses use it to explain why deeper burns need medical care, and forensic medicine classes use it when discussing burns and scalds and how burns made in life differ from those made after death.

Frequently Asked Questions

Why are full-thickness burns sometimes painless?
Because the burn has destroyed the nerve endings in the skin. Lack of pain therefore does not mean a burn is minor — one of the key points the model helps to teach.
